Nearly 3,500 babies die each year in the US from sleep-related causes, according to the CDC. The American Academy of Pediatrics has developed evidence-based safe-sleep guidelines specifically designed to reduce this number. Implementing these guidelines correctly in your crib setup is one of the most important things you will do as a new parent — and it is also one of the most frequently done incorrectly, often because well-meaning relatives give crib bedding sets with bumpers, or because marketing language obscures what "safe" actually means.

This checklist walks through every element of an AAP-approved crib setup so you can verify your baby's sleep environment before the first night home.

The AAP Safe-Sleep Rules: The Non-Negotiables

The American Academy of Pediatrics publishes and regularly updates its safe-sleep guidelines. The core requirements for crib setup are:

  1. Back to sleep, every sleep. Every nap, every night. Until baby can roll from back to front AND front to back independently, always place baby on their back. Once a baby can roll both ways, you do not need to reposition them if they roll in sleep.
  2. Firm, flat sleep surface. A CPSC-approved crib, bassinet, or play yard with a firm, flat mattress that fits snugly in the frame. No inclined sleepers. No bouncers. No car seats as sleep spaces outside the car.
  3. Tight-fitting fitted sheet only. No loose blankets, no pillows, no positioners, no stuffed animals, no bumpers of any kind — padded, mesh, or otherwise.
  4. Room sharing without bed sharing for at least the first 6 months, ideally the first year. Baby in the same room as parents, in their own approved sleep space.
  5. No bed sharing. The AAP is explicit: sharing an adult sleep surface with an infant significantly increases the risk of sleep-related infant death.

The Complete Crib Safety Checklist

The Crib Frame Itself

  • [ ] Crib was manufactured after June 2011 (when CPSC banned drop-side cribs and implemented stronger structural standards)
  • [ ] Slat spacing is no more than 2 and 3/8 inches — test by verifying a soda can cannot fit between slats
  • [ ] No broken, missing, or cracked slats or hardware
  • [ ] All assembly hardware is present and tightened — no wiggle in the frame structure
  • [ ] Crib is recalled-free — check CPSC.gov with model number before use
  • [ ] No corner posts extending above the crib frame rail (entrapment hazard)
  • [ ] Mattress support is at the appropriate height for baby's current developmental stage

The Crib Mattress

  • [ ] Mattress is firm — pressing the center with a hand should not leave a lasting impression
  • [ ] Mattress fits snugly in the crib — no more than two finger-widths of space between mattress edge and frame on any side
  • [ ] Mattress is flat — no shaped, contoured, or angled surfaces
  • [ ] Mattress is new or confirmed clean and uncompromised if secondhand (the AAP recommends purchasing a new mattress even when using a secondhand crib frame)
  • [ ] Mattress meets ASTM International crib mattress safety standards

The Sleep Surface

  • [ ] One fitted crib sheet only — designed for the specific mattress dimensions
  • [ ] Sheet fits snugly at all corners without any loose material
  • [ ] Waterproof mattress protector (optional, but if used: firm-fitting, not quilted or padded)
  • [ ] No bumpers of any kind — padded, mesh, or "breathable"
  • [ ] No pillows
  • [ ] No loose blankets
  • [ ] No stuffed animals or toys
  • [ ] No positioning wedges, sleep positioners, or anti-roll devices
  • [ ] No sleep props that alter the flat surface

The Baby's Clothing and Temperature

  • [ ] Baby dressed in appropriate layers for room temperature (68 to 72°F is the AAP-recommended nursery temperature range)
  • [ ] Sleep sack used instead of a loose blanket — appropriate TOG rating for room temperature
  • [ ] No hat worn during sleep indoors (overheating risk)
  • [ ] Baby does not feel hot to the touch on the chest or back of neck

The Room Environment

  • [ ] Baby is in the same room as parents for sleep (room sharing) — for at least the first 6 months
  • [ ] Baby is NOT in the parents' bed (no bed sharing)
  • [ ] No cords from monitors, curtains, or blinds within reach of the crib
  • [ ] Crib is not positioned near a window with direct draft or sun
  • [ ] No heavy items on shelves above the crib that could fall
  • [ ] Room temperature is comfortable — 68 to 72°F
  • [ ] White noise machine is at least 7 feet from baby and below 50 dB if used

Selecting an AAP-Compliant Crib Mattress

The mattress is the single most safety-critical nursery purchase. A crib mattress must be firm, flat, and sized to fit the specific crib frame with no more than two finger-widths of gap on any side. Look for:

  • ASTM F2933 certification — the standard for full-size crib mattresses
  • ASTM F2929 for non-full-size cribs and bassinets
  • GREENGUARD Gold or GOTS organic certification if chemical exposure is a concern
  • Firmness that does not compress under hand pressure at the center
  • No memory foam in the infant sleep surface layer (memory foam allows face-sink)

Dual-firmness crib mattresses (firmer on the infant side, slightly softer on the toddler side) are a practical option — the infant side meets AAP firmness requirements; the toddler side is appropriate after age 12 months when the sleep surface risk profile changes. Use our mattress firmness matcher to verify that your current or prospective crib mattress meets AAP standards for your baby's age.

Adjusting the Mattress Height: A Timeline

Most cribs offer 2 to 4 mattress height positions. The highest position is appropriate for newborns who cannot move independently, and it reduces how far parents need to bend to place and retrieve baby. As baby develops:

  • Lower one position when baby begins to sit up independently — typically 5 to 7 months
  • Lower to the lowest position when baby can pull to a standing position — typically 7 to 10 months

At the lowest position, the top crib rail should be at least 26 inches above the mattress surface, preventing a standing toddler from toppling over. Do not wait until after baby can stand to lower the mattress — the adjustment should happen before the skill fully develops, not after. It takes approximately 5 minutes and prevents a potentially serious fall.

Secondhand Cribs: The Safety Protocol

Using a secondhand crib is acceptable with the following confirmed: manufactured after June 2011 (when the CPSC strengthened standards and banned drop-sides), no missing or cracked hardware, slat spacing of 2 and 3/8 inches or less, no outstanding recalls (check CPSC.gov). Always purchase a new mattress for a secondhand crib — used mattresses may have compressed, contaminated, or compromised foam that does not provide adequate firmness, and there is some research suggesting that used mattresses from different households are associated with elevated SIDS risk.

What to Do With Gifted Bumper Sets

Receiving a crib bedding set with bumpers as a baby shower gift is common — bumper sets are still sold and gifted despite the CPSC ban because the ban was phased in and because many gift-givers are not aware of current safety guidance. When you receive such a gift: thank the giver, use the sheet and any unfitted accessories like the decorative skirt that hangs outside the crib, and remove and discard (or donate to textile recycling) the bumper panels. This is not optional — the bumper should not be stored in a closet "in case" or used later. It should be removed from the nursery entirely.
