Parents spend significant time comparing crib mattress features โ€” organic materials, breathability, certification labels, price points. These factors matter, but they are secondary to one non-negotiable requirement: the mattress must be firm. Not "medium." Not "supportive." Firm โ€” specifically defined by the AAP as a surface that does not noticeably indent when pressed and springs back immediately when released.

This guide explains why firmness is the primary safety specification, how to test any mattress you are considering, which types pass and which do not, and what to do with a mattress you are not sure about.

The Connection Between Mattress Softness and SIDS Risk

The American Academy of Pediatrics 2022 safe sleep guidelines list soft sleep surfaces as a separate, independent risk factor for SIDS and sleep-related infant deaths. This is not a general recommendation for infant comfort โ€” it is a specific risk-reduction measure backed by decades of research.

The mechanism: an infant who rolls to or is placed face-down on a soft surface can create a face-shaped indentation in the material. Exhaled carbon dioxide โ€” which is heavier than ambient air โ€” can accumulate in this pocket rather than dispersing into the room. The infant then rebreathes air with elevated carbon dioxide concentration and reduced oxygen. In most healthy, developmentally mature individuals, this triggers an arousal response โ€” the person wakes up and repositions. In infants, particularly in the first 6 months when arousal responses are less reliable, this mechanism can fail.

This is why back sleeping, firm surfaces, and no soft objects are the three most consistently emphasized AAP safe sleep factors โ€” they address the same physiological vulnerability from different angles.

What "Firm" Means in Practice

The AAP defines a firm sleep surface as one that does not indent noticeably when the infant is lying on it. In practice, this means:

  • The palm test: Press your full palm into the center of the mattress and hold for 5 seconds. Release. The mattress should return to its original flat position within 1 to 2 seconds. No handprint should remain.
  • The two-finger test: Press two fingers firmly into the mattress surface. The resistance should be immediate โ€” you should feel substantial pushback almost instantly. The surface should not easily compress under the force of two fingers.
  • The visual check: When the baby is lying on the mattress, there should be no visible sagging or sinking around the body. The mattress surface should appear flat and level.

A firm crib mattress will feel hard to you as an adult. This is correct. Infant sleep requirements are the opposite of adult sleep preferences. A mattress that feels appropriately supportive to an adult is likely too soft for an infant. Apply this to every mattress evaluation โ€” if it feels good to you, it may not be appropriate for your newborn.

Types of Crib Mattresses and Firmness Profiles

Foam Mattresses

Foam mattresses are the most common and most affordable crib mattress type ($50 to $150 for most models). High-density polyurethane foam mattresses can be appropriately firm and pass AAP guidelines. The key is density โ€” measured in pounds per cubic foot (PCF). Look for infant-side foam with at least 1.5 PCF density; quality infant mattresses typically have 1.5 to 2.0 PCF. Low-density foam (below 1.0 PCF) compresses too easily.

What to avoid: any foam marketed as memory foam, viscoelastic foam, or temperature-sensitive foam. These are specifically designed to conform to and retain body impressions โ€” failing the firmness test by design. Also avoid foam mattresses with pillow tops, quilted surfaces, or soft covering layers that add softness above the foam core.

Coil/Innerspring Mattresses

Coil mattresses use a spring core covered by foam or fiber padding layers. They can be extremely firm depending on coil count and padding thickness. Higher coil counts (135 to 260 coils in a standard crib mattress) generally mean better support and durability. The critical check is the padding layers โ€” some coil mattresses have thick soft foam or fiber padding on top that creates a softer overall feel despite the firm core. Apply the palm test to the actual sleeping surface, not just the mattress construction.

Coil mattresses are generally heavier and more durable than foam, and some dual-firmness coil options maintain firm infant surfaces well through several years of use.

Organic and Natural Fiber Mattresses

Organic mattresses use materials like GOLS-certified organic latex, organic cotton batting, or natural wool as alternatives to synthetic foam. Quality organic options can be very firm and meet AAP guidelines โ€” but "organic" does not automatically mean "firm." Apply the palm test to any organic mattress just as you would to a foam option. Some organic latex mattresses have a softer feel that does not meet infant firmness requirements.

The primary advantage of organic mattresses is reduced chemical exposure โ€” lower VOC off-gassing compared to synthetic foam, and OEKO-TEX or GOTS certification indicating material testing. For families with concern about chemical sensitivity or who want to minimize synthetic material contact, organic options are worth the premium price.

The Newton Wovenaire Mattress

The Newton uses a fundamentally different material โ€” a 90%-air, food-grade polymer structure rather than foam, coil, or natural fiber. It is designed to be firm enough to meet safety standards while providing airflow through the material itself. Independent testing supports its firmness claims. The breathability is a genuine differentiator โ€” if a baby presses their face into the surface, air can still move through the material. At $299 to $329, it is a premium investment, but the engineering rationale is sound.

What to Never Add on Top of a Crib Mattress

A firm crib mattress becomes unsafe if anything soft is placed on top of it. This includes:

  • Mattress toppers of any material (foam, cotton, wool, fiberfill)
  • Quilted mattress pads with significant thickness
  • Sheepskin or lambskin liners
  • Extra blankets or comforters laid flat as a "soft layer"
  • Any product marketed as "comfort layer" or "sleep enhancer" for infant cribs

The only addition on the mattress surface that is AAP-compatible is a single tight-fitted sheet with no excess fabric. The sheet should fit tightly enough that it cannot be pulled away from the corners. Waterproof mattress covers (thin, fitted โ€” not padded) can be used underneath the sheet to protect the mattress. The key word is "thin" โ€” a padded mattress protector adds soft surface area and is not appropriate.

Secondhand Crib Mattresses: The Safety Considerations

The AAP recommends caution with secondhand crib mattresses. The concerns:

  • Unknown compression history: A foam or coil mattress that has been used for 2 to 3 years may have compressed significantly from its original firmness, even if it appears intact
  • Mold or bacteria in the core: Moisture from spit-up, sweat, and diaper leaks can penetrate covers and colonize mattress cores in ways that are not visible from outside
  • Cover damage: Compromised waterproofing allows further moisture infiltration

If accepting a secondhand mattress: apply the palm test rigorously, inspect the cover for any tears or damage, confirm it was purchased new by the previous owner and used for no more than one child, and ensure it has been stored in a clean, dry environment. When in doubt, a new budget foam mattress at $60 to $80 is the lower-risk choice.

Certifications to Look For

While no certification guarantees AAP firmness compliance (always test with your hand), these certifications indicate other relevant safety factors:

  • GREENGUARD Gold: Low VOC chemical emissions โ€” particularly relevant for foam mattresses
  • GOLS (Global Organic Latex Standard): Organic certification for latex-based mattresses
  • GOTS (Global Organic Textile Standard): Organic certification for cotton batting and fiber materials
  • OEKO-TEX Standard 100: Tests for harmful chemicals in textile components
  • CPSC compliance: Required baseline for any crib mattress sold in the United States
